Ankyloglossia, commonly known as tongue-tie, is a congenital condition that limits the tongue’s ability to move freely. The condition is caused by an abnormally short, thick or tight lingual frenulum, which connects the bottom of the tongue to the floor of the mouth. Breastfeeding can be a challenge for babies with tongue-tie, as they may have difficulty latching on.

Additionally, tongue-tie can cause difficulty in speaking, eating, and swallowing. While some cases of tongue-tie are asymptomatic, others may require a corrective surgical procedure. The cause of tongue-tie is largely unknown, although there are some genetic factors that have been associated with the condition. Typically, the lingual frenulum separates before birth, allowing for the tongue’s unrestricted range of motion.

The treatment for tongue-tie remains a matter of debate among medical professionals. While some recommend immediate correction, others opt for a wait-and-see approach, allowing time for the lingual frenulum to loosen on its own or for tongue-tie to persist without causing any issues. Seeking guidance from a lactation consultant may assist with breast-feeding, while speech therapy from a pathologist can enhance speech sounds.

In some cases, surgical intervention, such as a frenotomy or frenuloplasty, may be necessary for infants, children, or adults who experience difficulties due to tongue-tie.

Frenotomy

A surgical procedure known as a frenotomy can be performed with or without anesthesia. During the procedure, the lingual frenulum is carefully examined by the doctor and subsequently freed by means of sterile scissors. As there are only a few nerve endings and blood vessels in the lingual frenulum, the procedure is usually painless and brief, with little to no discomfort for the baby. Bleeding is unlikely to occur, but if it does, it will only be minimal and consist of a few drops. Following the procedure, the baby can resume breastfeeding immediately.

Frenuloplasty

If additional repair or the lingual frenulum is too thick for a frenotomy, a more extensive procedure called frenuloplasty may be advised. This procedure requires general anesthesia and surgical tools to release the frenulum, after which sutures are used to close the wound.
